Anonymous Survey Consent
You are invited to participate in a research project entitled Attitudes and Barriers Toward Seeking Treatment Within College Athletes designed to analyze the correlation between mental health and sports and how they affect or influence barriers towards treatment. The study is being conducted by Zach Garrett as the principal investigator and Kyla Lett as co-investigator from Marshall University and has been approved by the Marshall University Institutional Review Board (IRB). This research is being conducted as part of the Research Methods class from the Professional Master of Science Athletic Training (PMSAT) program for Kyla Lett.
This survey is comprised of the most recent form of the Sport Mental Health Assessment Tool 1 (SMHAT-1), developed by the International Olympic Committee, the General Help Seeking Questionnaire (GHSQ), and the Attitudes Toward Seeking Professional Psychological Help-Short Form (ATSPPH-SF). This survey will take approximately 10 minutes to complete. Your replies will be anonymous, so do not type your name anywhere on the form. There are no known risks involved with this study. Participation is completely voluntary and there will be no penalty or loss of benefits if you choose to not participate in this research study or to withdraw. If you choose not to participate you can leave the survey site. You may choose to not answer any question by simply leaving it blank. Once you complete the survey you can delete your browsing history for added security. Completing the on-line survey indicates your consent for use of the answers you supply.
